What services does Care Plus provide?

We offer two types of services:

1. Skilled Home Health Services (Medicare-certified):

  • Skilled Nursing
  • Physical Therapy (PT)
  • Occupational Therapy (OT)
  • Speech Therapy (ST)
  • Medical Social Work

2. Private Duty Services (Non-Medical Support):

  • Personal care (bathing, grooming, hygiene)
  • Companionship
  • Medication reminders
  • Meal preparation
  • Light housekeeping
  • Mobility and vital sign support

What will my insurance cover?

If approved, your insurance will cover:

  • 100% of skilled nursing and therapy visits (PT, OT, ST)
  • 100% of the nursing assessment fee

This means no out-of-pocket costs for medically necessary skilled services—so you can focus on recovery and well-being.

What if I want to pay out-of-pocket?

We also offer private pay options for those without coverage or who prefer to pay directly:

Private Duty (Home Health Aide):

  • $40/hour (1-hour minimum)
  • $200 nursing assessment every 6 months
  • Monthly billing by Care Plus Home Health Care

Skilled Services (Private Pay):

  • $200 initial evaluation per discipline (nursing, PT, OT, ST)
  • Per-visit rates:
    • Skilled Nursing – $100
    • Physical Therapy – $140
    • Occupational Therapy – $120
    • Speech Therapy – $140
  • Monthly billing by Care Plus

How often will a caregiver or therapist visit?

Visit frequency is based on your personalized care plan. Some individuals may need daily visits, while others may only require support a few times per week.

How do I qualify for services?

To receive home health services under Medicare or insurance, you must meet homebound status criteria, which typically include:

  • Needing help to leave home (another person, walker, cane, wheelchair, etc.)
  • Leaving home is a taxing effort or occurs infrequently
  • Unable to safely leave due to illness, injury, or cognitive limitations

You do not have to be confined to your home—just that leaving is difficult and requires support.

What is the typical care timeline?

Here’s what the care journey may look like:

  • Hospital or ER Visit → Short-Term Rehab/Health Center Stay → Return Home with Care Plus Services (Medicare Part A coverage) → Outpatient Therapy → Ongoing Wellness Support
