Our therapy team provides up to 4 weeks of care, frequency depends on individual needs

2. Private Duty Services

• Light housekeeping
• Companionship
• Meal preparation and assistance
• Medication reminders
• Vital sign monitoring
• Mobility support
• Personal care (bathing, grooming, hygiene)

We offer care that patients can choose from to fit their needs and schedule. Cover depends on the availability of staff.

If approved and depending on your plan, your insurance will cover 100% of the nursing assessment fee as well as all skilled nursing and therapy visits—including physical and occupational therapy. This allows you to utilize our services without worrying about out-of-pocket costs, so you can focus on your recovery and well-being.

To receive home health services under Medicare or insurance, you must meet homebound status criteria, which typically include:

• Needing help to leave home (another person, walker, cane, wheelchair, etc.)
• Leaving home is a taxing effort or occurs infrequently
• Unable to safely leave due to illness, injury, or cognitive limitations

You do not have to be confined to your home—just that leaving is difficult and requires support.

Here’s what the care journey may look like:

• Hospital or ER Visit → Short-Term Rehab/Health Center Stay → Return Home with Care Plus Services (Medicare Part A coverage) → Outpatient Therapy → Ongoing Wellness Support

The client will be billed per hour for personal care assistance. Additionally, and ongoing, a nursing assessment fee will be paid by the client every 6 months. A registered nurse will assess the client upon admission and every 60 days to update the plan of care as needed.

Invoices shall be issued by Care Plus and are payable by the client on a monthly basis.
