Care Plus is dedicated to providing the full continuum of care allowing you to remain independent and recover in the comfort and privacy of your home.  We offer comprehensive services provided by licensed nurses, therapists, and home health aides under the direction of a physician’s order.

 

Eligibility Criteria for Home Health:

Eligibility for home health services may be determined by medical necessity and prescribed by a healthcare professional. To qualify for home health services, the patient must be homebound and have a need for skilled care.   Home health care services are covered under Medicare Part A and Part B. Private insurance may have its own eligibility criteria.

 

Medicare will pay for home care visits when the following criteria have been met:

Physician Order: A physician certifies there is a need for home health services.  Documentation supports the individual’s homebound status and the need for skilled services during in-person visits.  An in-person visit must occur 90 days prior to the start of home health services or 30 days after.

Homebound Status: The patient must be homebound. This means that it is extremely difficult for the patient to leave their home, and they require assistance to do so safely. Homebound status doesn’t mean that the patient never leaves their home but rather that leaving the home requires a taxing effort.

  • Is unable to leave the home due to an illness or injury
  • Experiences a considerable and taxing effort to leave and is absent from the home infrequently for a short duration or to receive medical care or attend religious services.
  • Has a functional or mental deficits impacting patients’ ability to leave home
  • Requires the assistance of a device, other persons or special transportation to leave home
  • Requires use of a device to leave home (crutches/cane/wheelchair/walker/etc.)

Need for Skilled Nursing Services: The patient must have a medical condition or need that requires skilled nursing care or therapy services. These services must be reasonable and necessary to treat the patient’s illness or injury.  A need may occur due to one or more of the following:

  • Recent Hospitalization
  • Recent new or exacerbated diagnosis
  • Change in medications in the last 60 days
  • New medication in the last 30 days
  • Change in primary caregiver providing care with a knowledge deficiency

Skilled Nursing Services performed by a nurse include:

  • After-Surgery Care
  • Medication Management
  • Wound Care & Dressing
  • Catheter Care
  • IV Therapy
  • Tube Feedings
  • Disease Management
  • Diabetic Care
